#EPeeps Would love to hear different opinions on what to do with 45 day TEE for pts that recently underwent LAAC. While on one hand TEE has high risk of exposing HCPs and patient, on the other hand, for a lot of patients at high bleeding risk rescheduling may cause bad outcomes.

@josoriomd The first part of the tweet got cut off but basically the origin of the 45 day TE was simply to see if the device migrated in the original trials and to see if there was a rhombus. Low incidence of both of original implant was good

@josoriomd here is very little chance of device migration that would be significant enough to warrant continued anticoagulation. Also the patients typically have been on anticoagulation for the first 45 days so it is fairly unusual to see thrombus there. The protocol since commercial launch

@josoriomd that we have done here at Pacific Heart and at Mount Sinai is 45 days of anticoagulation with antiplatelet followed by 45 days of doing antiplatelet then aspirin alone. We actually do the transesophageal echo at four months post implant when they're on aspirin alone.

@josoriomd Would bypass the 45 day TEE. We have already presented our data on over 500 patients will be bypassed the 45 DTE. If you actually look at the origin of why we were doing this test in the early days was really to see if the device migrated. We know now that if the implant is good
