@ColeyshawGemma We don’t have a dedicated team, with the support of @iDH_swb our therapist goes out. We don’t hold the acute bed for them but we do always have a back up plan incase we need to step them up to a pathway 2 bed / get urgent care or equipment. 😊

@SWB_RRTS Do you have a dedicated #DischargeToAssess team in the hospital or is it the rapid response therapists working on wards that would go out & do this Ax? And out of interest do you hold the bed incase the d/c Ax is not successful?

I had the privilege of seeing one of your therapists talking to a patient in our very busy ED on Tuesday. The care and attention she gave above the noise and the bustle trying to make the lady feel like she was the only patient that mattered. This really is an exceptional team.
