Congresswoman Lauren Underwood recieved degrees in public health and her medical career was focused on policymaking rather than clinical treatment, but everybody (correctly) refers to her as a “Nurse,” and that’s fine.

El-Sayed is a Rhodes Scholar who attended University of Michigan Medical School and subsequently completed an MD at Columbia University, was an Ivy League professor of Epidemiology, the Executive Director of the Detroit Health Department.
I think most people are fine with shorthanding that as being a “doctor.”
