@tomgoom @TheHipPhysio This is great, I like the reference to gynae problems, this is definitely a consideration. A family member had a rare congenital gynae pathology diagnosed this year after I pushed hard to advocate that it was not MSK. Symptoms were low back and buttock pain especially night time.

Our A Level story. My lad got ABC. I sang Jackson 5. He wasn’t impressed. He got zero offers for physiotherapy 💔 He also was a pallbearer for my dad in Jan, saw his dad leave to go overseas in Apr. Still, he rose. I’m so fkn proud. He’s asked me for a year off. Uni can wait.

@jim_crawfurd @BrigantiPhysio @DrNeenaJha @neiljlangridge In reality it works well, so long as good surgery staff especially care navigators understand the role. I rarely need to direct patients onto GP but they are always there if I need. Much less that I identify need GP than come to me from a GP for second opinion. Its 2 way street!

Getting a lot of anger & backlash against this tweet from physiotherapy colleagues Firstly, I apologise if I’ve offended anyone, that was not my intention in the slightest. Throughout all messages I’ve emphasised how much I respect & appreciate the training physiotherapists undergo. I’ve learnt a lot from physios & their expertise & knowledge of MSK conditions - we refer to them for their expert opinion & input. The ONLY difference of opinion was for patients presenting with acute undifferentiated symptoms without a preceding hx of trauma - I feel as a GP, they should be triaged by a GP first rather than patients directly booking in a physio clinic. I’ve had many patients with acute ovarian pathology, gallstones & angina presenting with new onset shoulder pain - many of these patients assumed it was muscular in cause due to the location of pain. This is not saying I don’t believe physios cannot diagnose these conditions - just simply in these instances patients should be managed by a GP as per our job & training too in managing atypical presentations of medical conditions This does not in any way discount the contribution/importance of physiotherapists.

The only time I feel physiotherapists can be first contact is if there is a very clear history of injury - e.g. if a patient twisted their ankle For undiagnosed & first presentations of pain e.g hip pain, shoulder pain etc they should all be triaged by a doctor first as there are non MSK red flag causes that must be excluded & delays in diagnosis can have a huge impact. Unfortunately I’ve witnessed this in practice

@BrigantiPhysio @DrNeenaJha @neiljlangridge FCPs alongside our GP colleagues are vital to ensure sustainability of the NHS….not forgetting that MSK problems are the second highest reason to book an appointment and constitute 25-30% of visits!

@DrNeenaJha @neiljlangridge More gps tomorrow would you be able to fill with gps is there 1000s out there. It needs funding to get more studying medicine and specialize in primary care and then gps can see every patient if they so chose but for majority of msk conditions FCP is vital to manage these.
