What's unfortunately about the management of autoinflammatory diseases is that I see many rheumatologists that feel completely uncomfortable in their management, even after the patient has a genetically confirmed diagnosis. Many won't even help the patient get on anakinra (Kineret) or canakinumab (Ilaris), which tend to be the mainstay of treatment for most of these patients. Biggest teaching point I'd have for anyone listening is to not be afraid to use higher doses of these - especially anakinra. You should be comfortable having your patients use as much as 900mg/day (no typo) as an outpatient. (I wish it came in at least 300mg syringes). As an inpatient, for severe flares, working with your pharmacist to run a continuous 24 hour infusion of anakinra at doses of 600-1200mg/24hr is a great way to treat severe flares. (also works *extremely* well for macrophage activation syndrome [MAS]/hemophagocytic lymphohistiocytosis [HLH]).

@psufka @rheum_cat Would love to see you work with @AAlliance_SAID to see if we could get an Autoinflammatory keynote at ACR24 to raise awareness among rheumatologists. It’s truly sad that only 1% of rheumatologists worldwide are aware of these diseases when they’re supposed to be the experts. 🙏🏻

@SameerudDowla @AlonLeeGreen But "Black Lives Matter" can't be interpreted as pro-ethnic cleansing. "From the river to the sea" means Israel no longer exists, and Jewish residents are evicted/killed. There are better ways to advocate for Palestinians that don't imply ethnic cleansing. adl.org/resources/blog…

It needs to be said: We, the Jewish people who live in Israel, are not going anywhere. Too often we here calls to "go back where we came from", or "from the river to the sea..", and the only answer to that is the simple fact that millions of Jews are going to remain on this land.

@SameerudDowla @AlonLeeGreen This is comparable to the Confederate flag. In a 2020 poll, many ppl said it's a symbol of "Southern pride" and not racist. But it's still an offensive/racist symbol, even if the meaning is different to some. Saying otherwise normalizes racism (or, for this phrase, anti-Semitism)

Appalling videos from Makhachkala, Russia, where an angry mob broke into the airport searching for Israeli citizens on the flight from Tel-Aviv. This is not an isolated incident in Makhachkala, but rather part of Russia’s widespread culture of hatred toward other nations, which is propagated by state television, pundits, and authorities. The Russian foreign minister has made a series of antisemitic remarks in the last year. The Russian President also used antisemitic slurs. For Russian propaganda talking heads on official television, hate rhetoric is routine. Even the most recent Middle East escalation prompted antisemitic statements from Russian ideologists. Russian antisemitism and hatred toward other nations are systemic and deeply rooted. Hatred is what drives aggression and terror. We must all work together to oppose hatred.

Cornell is aware of the rape and death threats Jewish students have received, and the police are investigating them. These universities (Cornell, Columbia, Harvard…) are solely responsible for allowing a climate in which Jewish students are afraid for their lives. Fix it.
