Community pharmacy staff often request hospital dispensing reports for safety net purposes. We want to avoid faxes. Many staff are not permitted to access MHR. Please don't ask us to send these to a @gmail account. What are the alternatives to support patients' continuum of care?

Our Muslim chaplain has been great - all about partnerships & ensuring patients make informed choices. 10% of patients reported medication events with fasting e.g. hypo with insulin, dehydration with diuretics. Tricky when patients refuse antibiotics, analgesia, asthma MDIs.

@ED_theEDpharm When we audited the reasons for patients refusing doses prescribed in hospital, we unexpectedly found fasting for Ramadan was a reason. When we interviewed our Muslim patients, they asked that we partner with them to come up with a plan and not simply tell them not to fast.
