Totally agree with this way of framing/thinking about $GUTS. Some ramblings building off of this--how I am currently thinking through GUTS commercial potential. x.com/pinker_ste9411… Of course there will be consumers that want to get a "one-and-done" (if it actually works). Obviously the durability data is going to need to be robust beyond 12-18 months--probably about 2 years of durability is where it starts to look really compelling for consumers and more importantly for payors/insurers. They will have something like 50 patients with 2-year data by mid-2027 (REMAIN + REVEAL cohorts), and then the ~350-patient pivotal cohort will have 2-year data around year-end 2027... For the first 6-12 months when this thing potentially gets approved (could be 1H27) it might be difficult to drive adoption and coverage from insurers. Assuming the durability plays out (which the limited data from their German trial suggests it should), the question becomes, like the post says, how many patients will they need to treat annually in order to make $GUTS a $1+ billion company. Backwards engineering and using the 3x sales rule of thumb, to get to ~$300 million of annual revs, with a $10k price tag per treatment (price used by Morgan Stanley in their model assumptions and which align with mgmt's guidance towards ~1-year injectable GLP-1 equivalent), you would need to treat around 30k patients per year. This seem feasible (for reference, Morgan Stanley has this going up to 100k+ patients per year in the 2030s). The 30k is just a fraction of the 120k+ bariatric surgeries that are quoted as being performed annually in recent years (pre-GLP-1 mania years, albeit), a fraction of the ~800k GLP-1-taking patients that receive routine endoscopies each year (according to the company), and obviously a small fraction of the number of patients that discontinue a GLP-1 each year. Overall, my current posture is that it might take a little time, but that $GUTS will eventually carve out a (potentially large) commercial niche.
