🚨The first American guidelines for low-dose radiotherapy in osteoarthritis were just published. That’s not a small thing. For decades, European centers have been treating OA with low-dose RT. American patients largely couldn’t access it. No formal guidance. No insurer framework. No roadmap for referring providers. That changes today. The American Radium Society Appropriate Use Criteria, published in the American Journal of Clinical Oncology, define exactly when and how to use LDRT for OA. Radiation oncologists, rheumatologists, physiatrists, and orthopedic surgeons built this together. For insurers, this is the clinical framework they’ve been waiting for. Honored to be a co-author alongside the global experts who made this possible.

Everytime I read FASTRACK I/II results I end feeling like we should be using SBRT for primary RCC much more, especially in older patients. Still, by using RECIST to define their local failure (≥20% size increase), they need a big change to achieve that definition +1 cm for a 5 cm mass. That growth takes some time even in untreated RCC. Distant control seems important here- and the freedom-from-metastasis curve looks really good. Would love to see that curve subsetted to their bigger masses (≥5cm or ≥4 cm). T1a (35% of cohort) unlikely to metastasize even with observation. Not sure if the T1b subset has ever been presented separately?

Many of them are straight up wrong as they aren’t peer reviewed or even created by an expert in that cancer type. Be CAREFUL when using these to learn about data and look at the source. #bcsm #breastcancer

1/ A very real problem in modern radiation oncology: The treatment plan is often highly detailed. The hospital record is often not. The TPS knows exactly what we treated, where we treated it, and to what dose. But much of that context never leaves the bunker. #ESTRO26 #RadOnc

6/ Radiology has PACS. Radiation oncology really does not. Our TPS does an amazing job planning dose. But we lack the hospital-wide communication layer that lets any clinician understand: What was treated? Where? To what dose? With what expected imaging change?
