(A 🧵 Thread) 11 Insurance Cancellations in 10 Years?! “I think I might have dozed off” – Richard Holden. The Story - On Sunday, September 28, 2025, a catastrophic crash occurred on northbound I-55 south of Festus, Jefferson County, Missouri, when the driver of a 2022 Freightliner Cascadia rear-ended a line of vehicles in a construction zone, triggering a chain-reaction collision involving eight vehicles (the semi and seven passenger vehicles). The driver, Richard Holden, was arrested and charged with three counts of involuntary mansl*ughter. Until now, we did not have the crash report or the carrier’s name. After receiving the report via an open records request, what we found is unforgivable and must be known to the public. The crash report led us to the carrier’s name, and we quickly pulled the records. Records reveal that the carrier, Transfinity Logistics, has four full pages of violations as of 10/31/2025. To put this in perspective, Transfinity has a fleet size of 30 units with 30 drivers (a small fleet). Insurance for a fleet such as this is at a minimum of $750,000. The Numbers - Their vehicle out-of-service rate was 36%. The national average is 23%. This makes them 55% worse than average. In 24 months, they received 60 total vehicle maintenance violations. This included brake violations, tire failures (sometimes with the fabric exposed or with sidewall bulges), suspension defects, and inoperable safety equipment. Their maintenance BASIC score was 6.51. This is very bad. However, somehow it got worse after looking at their driver out-of-service rate. The violations were very severe. They included operating without a valid CDL, overweight, and OOS violations. Recurring violations included false logs, concealed duty time, driving over the allotted time, driving without the required 30-minute breaks, and driving impaired due to fatigue. This carrier shows a widespread fatigue culture, which directly aligns with Holden's statement: “I think I might have dozed off.” The transport company’s constant insurance hopping is yet another red flag! Their insurance history is extremely unstable and shows a high-risk pattern. Records show 11+ insurance cancellations in 10 years. This is not normal! Multiple policies were canceled prematurely. Where was the accountability? The carrier had zero investigations because there was never a full compliance review. Compliance reviews are triggered by safety risk indicators, so why was this carrier never investigated? How did this slip through the cracks? This could have been prevented. We don’t just have an issue with illegals on the road, we have issues with those who open the transport companies and run the roads like a third world country.
