@bbcquestiontime @BBCOne Can the panel explain why crown immunity against health and safety prosecution still exists for prison service and government ministers? And if crown immunity ea to be lifted, do they believe the prison service would be forced to improve?

This week I was proud to present my Private Member's Bill: State-related Deaths (National Oversight Mechanism). It would fix a gaping and inexcusable hole in the government's responsibility to keep its citizens safe. Alexandra, a young woman in my constituency, died after public services repeatedly failed her. Her mum got in touch with me and told me her shocking story, and let me know about @INQUEST_ORG's #NoMoreDeaths campaign. When someone dies after contact with the state, hundreds of vital recommendations are made following inquests and inquiries. But unbelievably, it is not ANYONE'S job to collate these recommendations, analyse them for patterns, and make sure they get acted on. So it doesn't happen. Public and private bodies have a duty to keep us safe from harm and protect our lives, but every year hundreds of people die preventable state related deaths. The lack of any mechanism for learning from past tragedies is surely a big part of this. Let's fix this. Read more about the campaign: inquest.org.uk/no-more-deaths…

Today Green MP @carla_denyer will present a Private Members Bill to Parliament calling for the establishment of a National Oversight Mechanism. This is an important opportunity to further calls for independent oversight of recommendations following state-related deaths.

@carla_denyer @INQUEST_ORG The 1974 Health and Safety act gives immunity from prosecution to ministers and in turn, the prison services across the UK. Surely this needs to change if ministers and state run prison services are to be held accountable and be forced to make improvements.

@HRCScotland @UKNPM @ScotHumanRights @Jan_SHRC It is the exact same story with Scottish prisons. Scottish Mps and SPS hide behind immunity from prosecution while young people continue to die unessessarily. The only action that will instigate change is to remove prosecution immunity. (Westminter)

@shattenstone Let's hope he has the bravery to recommend the removal of immunity from prosecution under the Health and safety act that is afforded to Scottish ministers and to the Scottish Prison Service. It is ridiculous that this immunity still exists and the reason that nothing changes.
