Yes, a clean bathroom also shows they don't have many customers beyond the capacity which can't handled by the gym. When a gym has too many customers, it becomes too difficult to keep the equipment and the bathrooms clean and sanitized. The gym I have been using, the attendant sanitizes the equipment each time it is used by a customer. It also shows how he has been trained by the management to ensure safety of customers.

A rule of thumb when joining a new gym is to check the condition of its washrooms and changing rooms. If their washrooms are clean, by default, the gym is good and hygienic. If washrooms are dirty, it is fine to assume they didn’t clean and sanitise the rest of the equipment, benches, and mats.
