@2bz2 @olsonplanner It already is longer for the majority. Especially in surgery. Compare the amount of fellowships obtained now, before physicians begin independent practice, vs 10, 20, 30 years ago

@olsonplanner Necessity: residency is the last time a physician has a “safety net” of more experienced teachers. We learn our craft during this time. If it was not rigorous then it would need to be longer… time spent in training pays off once you are ALONE/ solo in practice: lonely out here

@JoelHillelsohn @SeanDreyer To build on this, reasons to suspect you’re still in the prostate are pain on inflation, and inability to irrigate any urine. You can also try flushing with 60ccs of NS. If you can flush and pull all 60 back you’re probably in the right spot. If you can’t pull 60 back, reposition

@SeanDreyer Algorithm: hold penis and stretch like you mean it. Try regular catheter first. If resistant at prostate switch to18 French coude. If there is distal resistance try a 12 French. Lastly never inflate till catheter is all the way in! You can get return of urine in the prostate.

You know how when several nurses can't get a foley in, then Urology gets paged to help... What does Urology do differently? Just want to know/learn the tricks. #UrologyTwitter #Urology someone?

@chloepetersMD @SeanDreyer To build on this, If they say they can put the catheter in, but it comes out of the vagina, it was likely always in the vagina and they are missing the urethra all together. Especially if no history of continuous urinary leakage or documentation of a fistula.

@SeanDreyer ♀️ tips cont: If RN says they can see the meatus but get immediate resistance, they may be trying to cath the clitoris. Have seen this multiple times. Review anatomy (kindly) and make sure they identify the clitoris above where they are trying to cath
