In May, 2020 at 9pm in A&E, I called a wife and daughter to tell them their beloved relative would soon die of #COVID, alone, distressed. They would never see him again. They pleaded to see him, but followed the rules. I still think about them. #BorisJohnson should be ashamed.

@agemjourney I remember being IMT2 post-PACES needing an ascitic drain signing off, spoke to the hepatology nurses who informed me that the word from the consultant supervising the unit was to prioritise their new PA so they could set up a new drain clinic...

Was writing discharge summaries when one of the ACPs came in and was taking about how they’d done 10 LPs but need 10 more for their log books & how they’d started management for quite a complex patient. Meanwhile I’ve just about managed to try two LPs in over a year 🙃

I'm aware this references Trump's decisions, but high-skilled workers and their recruitment is complex. Local markets of highly-skilled workers don't respond well to 'market pressure', with quality training opportunities a significant barrier in most healthcare systems.

RIP Charlie Kirk, horrific, words absolutely fail me. We must be allowed to disagree, to debate, to argue and be open-minded. We must be able to hear alternative and differing perspectives. Who brings a gun to word fight? I have never worried more for discourse in the USA...

What’s frustrating as a physician on social media is when you have fun content ideas to provide meaningful medical education to your audience, but then a powerful idiot starts mass diagnosing something fake like “mitochondrial challenges” while people watching at the airport, so then you’re like ok I guess I have to deal with this now

Fuck the NHS, as an employer, it's inability to evolve, the way it treats staff is with contempt. Train someone, and then fail to rehire. You cannot raise a family with the expectation of stability as a rotational doctor. It's shambolic.

I'm not talking about VTE audits, oxygen prescribing QIP, but ambitions in QIP for serious change, but it NEEDS senior leaders to drive change. The pain where I work now is palpable, but still, we need more, we demand more.

The NHS is chronically underfunded and that exacerbates the problem. We can do better though, we can drive change, why are we so unproductive now, in part, because the system has not improved.

One of the sad things about being a doctor in the NHS around rotation, is seeing how much we adapt to system inefficiency. A new doctor starts in a new trust and 'everything' falls apart. The quirky system, the weird things 'we just do' only ever survived by those adapted.

A colleague made a good point, when writing emails to 'all' think about the recipient more than yourself. NHS management gets the wrap of being tone deaf, it's true. Talk about things they care about, the change they want, not what you want to talk about ad nauseum.

If there is one toxic trait of NHS management I'd get rid of, it's the need to write trust-wide emails about 'the board'. I honestly couldn't give a toss how you arrange services, there is still no staff, no capacity and no goodwill. I fundamentally don't care.

The @RCPhysicians are a curious bunch, remember that the future (and current) members watch such behavior. I moved to @RCPEdin last year. The Leng review will comment on the RCPs biases, and as the years go forward, the halls will be as empty as their current policies. A shame!

FINALLY- Decision @RCPhysicians investigation Removal from the Council of an elected member Barring standing for elections 12 months By a panel which is unknown - with zero right to reply. (To be ratified by Council on 15/7) For exposing chicanery around PAs by senior execs While prev PRCP and Reg have zero sanctions after their actions brought along an EGM @djnicholl Pretty much as expected by an organisation trying to suppress any dissenting voice Regrets? Nil You can try whatever you want - the direction around PAs has changed for good Kids- never ever give up a fight when you are doing the right thing. On the upside? RCP have asked me to see my GP for mental distress this may have caused me. That’s nice- no? 😊

@worstall @DrLukeCraddock Is that fact or opinion? I know of a colleague who between PhD, maternity and a few years a locum SHO has only just CCT'd having graduated in 2010. Regardless, given prep time to medical school, as an SpR now I was influenced in my school years by the compensation back in 2008.

The hypocratic oath doesn't fly far these days, but it never makes sense to harm ourselves to protect patients. Training is exactly that. We deserve a system that appreciates all medical care is fundamentally guided by the hands of doctors, and that will never change.

If there is one promise I make to every junior doctor stick in the pathetic mess of medical education and training, is that we will do better when we rise to those powers. The current generation must improve on the failings of the past, for the real sake of patient.

I'm not an old timer, but even as a Specialist Registrar in Medicine I've noticed a change over my shortish career in the attitudes of Foundation Doctors. It used to be 'I want to do this' and 'I'm making a portfolio for that'. Now, it feels different.
