I think you may be correct about this point @jasonhoran. Patient visits to oncology daywards across the North-Northwest have doubled over the past 8 years. It seems there has been no forward planning for this. There are academic workforce planning projects afoot, but again, you cannot roster a hypothetical staff member. The reason I have noticed maternity leave backfill is that I am trying to identify any possible efficiency that could be made in the system to increase patient flow. Forward filling might work within a stable system, but with a doubling in demand (affecting nursing, medical, surgical, laboratory, nutrition, physiotherapy - the full spectrum), backfill of maternity leave should be actively considered.

@mccarthymt7 Our director of nursing says that when the staffing requirements for a unit/service are being calculated that predicted maternity leave is factored in at that stage. Essentially it is being "forward-filled" I suppose.

The more I discover about the @HSELive's "Pay and Numbers" strategy, the more disillusioned I become. As far as I can see, the only published reference to any details on this "strategy" is in this document: hse.ie/eng/services/p… It is entitled "Our National Service Plan 2024". The @hseNCCP recently developed the Systemic Anti-Cancer Therapy Model of Care: hse.ie/eng/services/l… A key principle of this strategy, in keeping with international @OECI_EEIG guidance, is that systemic therapy for cancer should be delivered closer to home. In other words, a cancer patient should not have to travel a long distance to get standard-of-care chemotherapy/immunotherapy. To deliver this strategy successfully requires specialist pharmacy, nursing, clinical nutrition, radiology, radiography, physiotherapy and medical care to be available in local model 3 hospitals. To recruit to specialist posts in model 3 hospitals can be challenging. The pay and numbers strategy, as far as I can see, and I am fully open to correction, effectively abolished once available specialist posts, that happened to be vacant in December 2023. It has been devastating. There is a very unhelpful and fully complete disconnect between service demand and service staffing at the heart of this strategy. It must end. I can understand crisis situations, but for the HSE to remain fully unaware (and seemingly uninterested in discovering) the difference between unnecessary posts, and critical posts that are difficult to recruit to- almost a year into the strategy - really makes me think. That Senior Management and Senior Administrative posts are explicitly exempt from the Pay and Numbers Strategy is almost unbelievable.

You can try to factor mat leave in but your application will be rejected. Currently it’s next to impossible to get paid mat leave backfilled in public health service for any profession, it’s not just confined to nursing.
