I hate to niggle, but I can't help it. That's not an "agent"
It's manual work chatting with an LLM.
That's of course some kind of improvement, but it's not sustainable.
The end goal is not having to prechart.
You show up to clinic and it has happened for you. You review a summary, and see the patient.
First experience with EPIC’s AI Summaries
“Generate Summary from Notes”
This was early access in my health system in the outpatient setting, where it can generate a summary of what's gone on with the patient since my last visit.
I'm overall pleased that Epic is headed in this direction, but so far my first impression was that it falls short in several major ways:
1. The latency essentially makes it unusable for me. It takes 40 seconds plus to generate a summary.
2. Summaries can only “see” notes. They don't see labs, they don't see imaging, and for a subspecialist that's severely limiting. Context is everything and that’s just not enough context.
3. The summary ends up showing me random snippets from my own previous note, and it's unclear how those are prioritized. They’re typically not the most important sentences.
In the 40 seconds it took to render, I'm best off just pulling up my last note and reading it myself.
Not quite ready for prime time, but a step in the right direction.
If you have a clinician-facing AI product, you need to think very carefully about the user intent. And it should be FAST. It cannot slow people down.

Even in a redo case of a 220-g prostate in a patient on Plavix and Aspirin, safe enucleation was achievable using a bipolar platform — 80-minute enucleation time with only a 1.5 g/dL hemoglobin drop.
@CanesDavid@UroOncologist True but ambient scribes have been readily adopted and they surely use more tokens to take in 10 min of speech and then generate a note vs taking in a note or two and some labs / imaging report
@UroOncologist It’s an internal decision probably, but every generated summary consumes LLM “tokens”
It’s not free to make a summary. Multiply that out across hundreds of thousands of visits you can see why they decide to make the MD click “generate summary” when needed.
@CanesDavid Haha true. Agent in name only. Cowork could do (in theory, not sure can interact w epic) but PHI to cloud bad. Best would be epic plugin / tool - seems so easy
@CanesDavid Just standard chatbot agent. Instructions like “take input (last progress note) and use to generate HPI for upcoming visit. Keep concise, maintain my writing style, keep relevant info and some formatting stuff
