Some of the people are gone, but the regime is still intact. Iran has over decades built a decentralized power structure where every individual leader has at least three people as backup to take over in the event some of them are removed. If you placed Iran over the USA, it would cover the distance from NY to Houston TX. It's of course smaller than the US, but also not something you can go to war with and win over a few days, or a couple of weeks.
