Reason # 65794 why #pocus is important: the initial diagnosis based on chest X-ray is often incorrect. Here is an elderly patient with history of heart failure with preserved ejection fraction admitted overnight with shortness of breath and hypoxia. The chest X-ray was read as “pulmonary vascular congestion” and she was started on diuretics for a presumed heart failure exacerbation. The following morning a #pocus trained doctor saw her. Here is the jugular vein exam with the head of bed at 45 degrees:
