@siamoxicillin Pet peeve of mine too. The "historian" is the one who documents the history, not the one who recounts it. It's not the patient's job to perfectly report every detail. It's our job to figure it out.

So I'm doing an educational module on blood transfusion, as required periodically by my hospital. When it comes to safety concerns, patients often ask about HIV. The risk of that is ~1 in 21 million.

💡azathioprine is metabolized primarily by TPMT (hence pre-Tx screening) and xanthine oxidase. Use of xanthine oxidase inhibitors (like allopurinol) will therefore ⬆️ azathioprine serum concentrations.

@DoubleDawgMD Thanks so much for this. One point of discussion: I find the nitrite helpful in the setting of patients who DO have lower urinary tract symptoms. If negative, it forces me to think about enterococcus, which wouldn't be covered by the cephalosporins that we often use. Thoughts?

@medstudent_kait If it’s mainly because of the people - is that a reason not to choose that specialty? The people you work with are one of the most important aspects of your career. If you like the people, ask them why they chose that specialty. You may find you relate to their answers.
