£16bn state housing bank. What could possibly go wrong? And how will it protect builders from war driven rising prices? Also a reminder that ‘affordable’ does NOT mean affordable except in the trivial sense that if a house is sold someone has afforded it.

Keto fans point to infants as proof that humans are “naturally ketogenic.” This sounds clever right up until you know any physiology. In utero, foetuses depend mainly on glucose crossing the placenta, and glucose is their primary fuel. Not quite the carnivore origin story. 🧵

The risk calculators severely underestimate risk. They are designed to keep more patients off medicine. As cardiologists, we see the reality. The calculators and guidelines take many years to catch up to the science. If your LDL-C is over 60, you are probably building plaque. That's why we want be super low. My LDL-C is 41 on crestor and zetia. And I would like to keep it that way. I would rather never have calcium, never have a stroke, never have a heart attack, never have any of these things. Do it now before it becomes irreversible! 💪🏻🫀🩺

Heating in the U.K. is a luxury item People in villages are absolutely hostage to the oil companies Here we don't have gas . To heat your homes and water , it's all oil. January the price of 500l was 62 a litre £310 Today it is 1.45 a litre 500l~ £761 ‼️‼️‼️‼️‼️ How are people expected to pay this on top of everything else!!!!!! DO NOT TAKE YOUR PHONE OUT IN PUBLIC IN CENTRAL LONDON! Was stood waiting to cross the road at corner Marble Arch and Oxford Street just now and turned to see bloke on very fast electric bike heading up behind me. I turned and he turned away. I stood and watched as it was obvious what was going to happen next and sure enough, he snatched the phone of a young woman stood at the bus stop. Then off down Oxford Street for more. Face fully covered. If we had a police force in London this could easily be stopped, but instead, dozens of people a day have their lives made shit by these vermin. I’m in a shop writing this. Put your phone away on the streets. Take care. Be safe.

Mozaffarian et al. (2010) is a meta-analysis of clinical trials that looked at the effects of saturated fats vs. PUFAs on cardiovascular disease. It is considered among the strongest pieces of evidence that dietary PUFAs, including n-6 PUFAs, are heart healthy. The main result, across 8 RCTs, is that increased PUFA intake is associated with a reduction in risk of cardiovascular diesease/death (Image 1). When you look at the meta-analysis and dig into the RCTs themselves, there are some intriguing things to note, including: - They include trials where "PUFAs" replace saturated fats, but these can include n3 or n6 PUFAs. The trials vary in what the experimental group actually consumes (seed oils, fish, etc). - The two largest trials, Minnesota CS and DART, do not show a significant reduction in CHD deaths (Image 1) - Some trials involve replacing animal fats (saturated) with vegetable/seed oils (n-6 PUFA), while others involve advising people to eat fatty fish (n-3 PUFA). In some trials, intake isn't really controlled; patients are "advised" on what to eat. - The Minnesota CS trial (n=9057; Image 2) shows no reduction in CHD deaths from increased PUFA intake. No difference in cardiovascular deaths or total mortality. Not clear what the sources of PUFAs were, but presumably some n-6 PUFA was included. - The DART trial (n=2033) did not appear to explicitly alter n-6 PUFA intake. Patients were men recovered from myocardial infraction and "advised" on diet. Subjects advised to eat fish had a 2-year all-cause mortality reduction (Image 3), which presumably would have meant an increase in n-3 PUFA intake. It could be fun digging into the other trials, and noting what kind of PUFAs were measured and how carefully intake was tracked.

@drterrysimpson they always seem to calculate LDL, if they bother at all. It isn't used in the risk calculator anyway. So I conclude it is irrelevant.

Your doctor measures cholesterol because it predicts who gets carried out of the hospital. Insulin, by contrast, mostly predicts who will buy a glucose monitor on the internet. LDL has causal evidence, randomized trials, and therapies that reduce heart attacks and death. Insulin has influencers, fasting curves, and arguments about what “optimal” even means. We don’t ignore insulin—we measure its consequences: glucose and HbA1c. Because medicine prefers endpoints over vibes. If insulin were as actionable as cholesterol, it would be on every lab panel already. It isn’t. That’s not a conspiracy—it’s a clue.
