With the great work of Christina Canavati, Ahmad Siam, Sapir Labes, Nirit Trabelsi, Elchanan Parnasa, Barhoum Barhoum, Nur Magadle, Ofer Perzon, Maya Blum, Ora Schueler-Furman, Yuval Tabach and Orly Elpeleg, all from Hadassah Medical Center @Hadassah_MC and the faculty of Medicine of the Hebrew University, @Hujimed @HebrewU_heb, we have published today in Circulation a research paper entitled: Pathogenic variants of scavenger receptor CD36 lead to decreased efferocytosis and predispose to myocarditis following vaccination with Pfizer-BioNTech BNT162b2 against coronavirus infection (COVID-19). Following the Authorization of the Pfizer-BioNTech BNT162b2 vaccine Israel initiated a nationwide vaccination campaign for COVID-19. the Pfizer-BioNTech BNT162b2 vaccine caused a higher incidence of myocarditis in young males (our three papers in @NEJM and Circulation). In the current study, we aimed to investigate the potential genetic predisposition to post-vaccination myocarditis. We performed whole-exome sequencing (WES) on a cohort of 16 male individuals, with a mean age of 26.5 years from three distinct ethnic groups, all of whom developed confirmed probable or definitive cases of myocarditis following their second dose of vaccination. We found likely pathogenic mutations in 50% (8/16) of the patients in the CD36, KCNQ1, TNFRSF13B, MEFV, and VCL genes. Of these mutations, 4/8 were variants located within the scavenger receptor, CD36, which is known to increase the risk of cardiomyopathy. Together with our recent paper showing HLA is important for the development of post vaccination myocarditis, it shows that genetic predisposition identifies persons with higher chances to develop post vaccination myocarditis.
