The community learning disability team in #NLincsisgreat attended the transition conference today held by Northern Lincolnshire and Goole, Humber Health Partnership, discussing how we can better support people with a learning disability transition into adult services.

Excellent conference today @NAViGOCARE 👏🏻 really emotive speakers and have learnt lots surrounding domestic abuse, forced marriage, child exploitation, county lines, modern slavery and honour-based violence which often affects patients with a known learning disability.

Thanks to the Endoscopy team at Grimsby hospital from a relative of a patient who said: "Mum has dementia so I was so worried she may become agitated but absolutely everyone spoke to her with such care and good humour, keeping her calm and reassured. #ThankYouTuesday
