This is an example of a "Rapid" ROP imaging technique that I use. In this example, I am using the PANOCam but this is repeatable on the other available machines. ROP (Retinopathy of Prematurity) screening is not comfortable for babies. This is also one reason why it is a difficult skill to teach - juniors are under pressure to complete the screen fast and seniors are under pressure to not put the baby through too much undue stress. It is common sense that a shorter screen, if possible, is always better for the baby. However, we have to balance this against being able to get the necessary information during the screen. I aim to: 1) minimise speculum time - a common error is to leave the speculum in whilst adjusting the focus. In this video, I adjust the focus whilst the speculum is out of the eye. 2) Assistant helps with the speculum and gel - I find this saves a good few seconds and the extra pair of hands can also help stabilise the eye. The assistant also opens and closes the lids during the anterior segment photos. This shortens the total speculum time. 3) for the retina, I am prioritising getting 5 photographs only. 1 of the posterior pole, and 1 of the temporal, nasal, superior, and inferior. In some scenarios, just the 3 may suffice (posterior pole, nasal and temporal) Sometimes, you do need to spend more time and collect more photos to make a monitoring/treatment decision but there are some scenarios where a few good quality photos are adequate. For example, - first screens - early period of stability following anti-vegf injections - very posterior ROP where most of it can be captured in a few frames It is also apt that these babies are the ones who will benefit most from a quick screen as they are likely to be sicker. For nurse-led photography which is more protocol based, it may be harder to go "fast" but for doctor-led photography where the decision making is done at the same time as the dynamic screening, rapid screens are more achievable. Interestingly, this screen was done in 30 seconds which is quite comparable to the gold standard Binocular Indirect Ophthalmoscopy. Of course, BIO vs retinal imaging for ROP is another topic in itself! Please share your thoughts if you have any! I look forward to sharing more videos using different retinal cameras.

We have a new slit lamp TV in our clinic room! Last year, I noticed that during maternity sonographer sessions, parents would be allowed to visualise the live ultrasound that was taking place. This was usually displayed on a TV in the room. Why not do the same for eyes? This is a beautiful way to deliver patient education and not to mention a much easier way to teach students observing in clinic! If you're a medical student would you find this useful, and if you were a parent or a patient would this be helpful for you? Also..Don't waste old tech! Millions of pounds are wasted daily when we get rid of perfectly functioning equipment. This is particularly prevalent in the NHS. Let's learn to repurpose old tech where possible. In this setup the microrec and ipod were graciously sponsored by the education fund but the AppleTv was donated by Ian, our lead optom and the TV was an old one from our meeting room that was going to be discarded.
