watchman today - got access and gave 150 units/kg heparin - TS with SL1 and Baylis - advancing watchman sheath and saw 1 cm clot adherent to RA side of septum - couldn’t aspirate so aborted case and resumed eliquis - has anyone seen this? @dngibson2

Clot around the deflectable sheath in the LA. 😱 Heparin bolus +aspiration dissolved clot. No neurological sequelae. #EPeeps

Pre-op venogram revealed a persistent left sided Superior Vena Cava. Surprisingly, it also revealed an absent right sided Superior Vena Cava. Couldn’t find many cases of this in the literature. #EPeeps #EP #CaseReport

What to do when you cant find the CS? Many have CS atresia with a persistent vein of Marshall (PVM) through which you can implant. Most people jut give up. I saw in two virgin implants in a row (not prior implant failures). If cant find CS dont waste time look for PVM #EPeeps
