What caught my eye is that medium SYNTAX score is no longer a predictor of MACCE!!! basically these are the patients that we offer them PCI or CABG (and PCI can be efficiently and safely done), while high SYNTAX are mostly CABG turndowns :(

@GreggWStone I think having ECMO in control arm will probably offset any mortality benefit of ECMO’s superior hemodynamic support (vascular complications, stroke,…), and IABP wouldn’t add much hemodynamic wise. I think the next RCT should be whenever 5L percutaneous pump is available

General cardiologists and interventional cardiologists only: Following Danger-Shock, would you enroll in a second trial of STEMI with cardiogenic shock in patients without anoxic encephalopathy, randomizing to Impella versus control (pressors +|- balloon pump +|- ECMO)?

Need the smart guys advice... 35.y.o healthy guy, no prior medical history. Chest pain and ST elevation on ECG (2 ECG here on admission and after 12 h). Normal coronaries per angio. No fever/uri/or any other complaints. Suppose to have MRI in the next couple of days. Any suggestions regarding the diagnosis (Trop I 40,000 on admission)... A patient who presented with St segment elevation (Ant + Inf) on ECG. On angio prox LAD lesion, underwent PCI. 2 days after, CAVB and CS. LVEF 35-39%. Underwent CRTD implantation and cardiac biopsy. What is the likely diagnosis? (With Prof. Tal Hassin).
