Early identification of people on #TimeCriticalMedication at ED reception is key to them being prescribed and administered within 30mins of when they are usually taken. @Leic_hospital ED reception staff have to complete a mandatory drop down list if they are @KirkwoodDW @FiMcD3

Clozapine-Induced Tachycardia 🫀🩺 Audit of medical files (101 patients) 👉52% of patients on clozapine had heart rates (HR) over 100 beats per minute (bpm), significantly exceeding previous reports. Clozapine's Unique Cardiovascular Effects 💊 Mechanism of Action: 👉Clozapine's specific effects on HR occur primarily through its anti-muscarinic and chronotropic properties ( via noradrenaline) 👉Dose-dependent HR increase 👉Clozapine : Norcloz ratio associated with ⬆️ risk of tachycardia ( Norcloz ⬇️ risk) Clinical implications : 🚨 👉Contrary to previous belief that clozapine-induced tachycardia is temporary, findings show that elevated HR can persist even after nearly a decade of treatment. 👉Persistent Tachycardia is associated ⬆️ risk of cardiac remodelling and cardiomyopathy 👉β-blockers are useful in lowering heart rates but they may be insufficient to reduce cardiac risk. 👉The study calls for a reevaluation of the conventional 100 bpm threshold for diagnosing tachycardia, especially in clozapine-treated patients. 👉Given the elevated average HR (about 20 bpm higher than population norms), there's a push to adjust guidelines to better reflect the risks associated with higher heart rates in psychiatric patients. @rcpsychTrainees Flexibility is key. Our systems are increasingly adapting to the needs of trainees beyond training alone, including a life that can be flexible around other factors. LTFT allows me to pursue what I love, in all parts of my life. 💞 No regrets. #ChoosePsychiatry
