Impressive case for #screenandintervene model for targeted #SDOH interventions in clinic. Eg sending Housing Advocacy letter to landlord (ex: mold ➡️ asthma exacerbation) resulted in 74% resolution of the housing issue. We should be empowered to "work up" social needs #SGIM24
