.@MEABharat Why renewal of passports in Australia takes time that too more than 3 months ? Posting on behalf of a friend ! @VFSGlobal takes more than a month to forward the applications !! Its high level of incompetence and brings more uncertainty for travel plans !

I agree and disagree with my old Bombay univ winning captain @sanjaymanjrekar . I agree: a 50 over World Cup win is like winning the soccer World Cup every four years which is why for me, nothing will match 1983 and Dhoni’s 2011 six . Can’t have the T 20 cycle every two years: that devalues a ‘World’ title. I disagree that T 20 wins don’t come ‘remotely close’: T 20 is flavour of gen next and a repeat win is remarkable. BUT, yes, for me, winning a 50 over World Cup is still the ultimate. As it is I am sure for most genuine cricket fans. (WTC also is very special but don’t agree with how points are counted). In my book 1) 50 overs World Cup 2) WTC 3) World T 20 (don’t call it a World Cup ) 4) Champions Trophy
Sanjay Manjrekar@sanjaymanjrekar

In time we need to put these world titles given out every year in proper perspective. India’s T20 WC wins don’t come remotely close to their 50 overs WC wins of 1983 under Kapil Dev & 2011 under Dhoni in terms of its pure challenge & it’s sanctity.

@sardesairajdeep @sanjaymanjrekar It is just that Having things in abundance often feels unimportant because human psychology is wired to assign value based on scarcity, utility, and novelty! Your mentality is getting shaped like this coz India is now winning more frequent as opposed to your times …

More evidence that #immunotherapy works better in the morning. In a prospective, randomized phase III trial, giving first-line chemo-immunotherapy earlier in the day (<15:00) nearly doubled PFS (11.3 vs 5.7 mo, HR 0.40) and improved OS (28.0 vs 16.8 mo; HR 0.42) in advanced NSCLC. More effective, no added cost and easy to implement. Likely reflects circadian immune rhythms with greater CD8⁺ activation and less T-cell exhaustion. doi.org/10.1038/s41591… @OncoAlert

The implications of these PFS data are huge given this was time limited therapy. If in remission around 1 year, you probably have at least a 50/50 chance of remaining in remission 4 years later off therapy! If this is what we see in 3L+, could anti-CD3xCD20 BsAbs be curative therapies for FL?

CRC before 50 isn’t just early - it’s different at the core 🧬🔥 📊 Large NGS study (n=1209; 4 Italian centers): • EO-CRC (≤50y; n=298) vs AO-CRC (n=911) • Profiling with FoundationOne CDx (324 genes) 🔍 Molecular fingerprints in EO-CRC: 📈 Amplifications: MYC, RAD21, GNAS, MAPK1 ❌ Loss: CDKN2B 🧪 Mutations: APC R876*, NRAS Q61L, PIK3CA E545K, TP53 G266V ⏳ Survival impact (PSM analysis): 📉 OS → 35 vs 41 mo (P=0.0326) 💪 ECOG 0 → 35 vs 44 mo (P=0.0026) ⚡ RAS/BRAF-mut → 27 vs 44 mo (P=0.0024) ⚖️ Take-home: EO-CRC is biologically distinct, molecularly aggressive, and demands age-tailored strategies. Not just coffee - even pembrolizumab works better before 11 AM ☕💉 📊 Eur J Cancer 2025 | 9-center retrospective | n=450 1L Pembrolizumab monotherapy in advanced NSCLC Timing groups: 🌅 Early ≤11:00 AM 🌙 Late >11:00 AM Results (PSM, n=206 | median f/u 62.5 mo): •OS: 43.7 vs 32.4 mo ➝ HR 0.67 (95% CI 0.46–0.97, p=0.03) ✅ •PFS: 13.8 vs 11.6 mo ➝ NS (HR 0.80) •irAEs ≥G3: 26.2% vs 13.6% (p=0.04) ⚠️ ✨ Take-home: First-dose timing may prime immune activation. Morning infusion → better OS, but higher severe irAEs. A simple, low-cost tweak worth prospective testing. 🔗 Eur J Cancer 2025 DOI: 10.1016/j.ejca.2025.115748 Full text: ejcancer.com/article/S0959-… #LungCancer #Immunotherapy #Chronotherapy #OncoTwitter @OncoAlert @OncoDailyNet @oncodaily @myesmo @asco

Academic medicine is bleeding talent. The best experts are counting to retire or switch careers at an alarming rate. It’s folly to think that you can replace top talent easily — you will need 10 people to provide the level of productivity of one top talent. The effects may not seem obvious. Because you seldom miss what you didn’t know you could achieve. But it will happen. In patient care, in research, and in the development of new treatments.
