Incorporating Quadruplets into Frontline Therapy for Myeloma " - Quadruplet therapy has been established as standard of care for fit, transplant eligible patients (PERSEUS, IsKIA, etc), and now emerging trials have demonstrated benefit of quadruplet therapy in transplant ineligible patients as well (IMROZ, CEPHEUS, BENEFIT) - The MIDAS trial uses MRD negativity to risk stratify patients into different consolidation arms (ASCT vs IsaKRd maintenance if MRD negativity; tandem vs single transplant if MRD+), and will help guide future decision making of omitting transplant in MRD negative patients - Future studies need to fill gaps for high risk populations (ultra-high risk MM, plasma cell leukemia and functional high risk) and to help answer questions about MRD guided treatment de-escalation" - Anupama Kumar #GDHEM #GreatDebates #Hematology #InsideInsights
