@arvindreddyt @ktkdoc @itisprashanth I don't think these protocols are even followed in hospitals, the people manning the mri are mostly disconnected from the treating doctor.

MRI is not a “follow SOP and finish” test. There are baseline protocols, but real diagnosis depends on what you add beyond that. From my own clinical practice, this is what actually happens: •Pediatric epilepsy case Child comes with “MRI brain normal.” On repeat with proper epilepsy protocol (thin slices, oblique coronal through hippocampus) → Focal cortical dysplasia identified. → Entire management changed. •Developmental delay / metabolic suspicion Routine MRI done with basic sequences → reported as “delayed myelination.” On targeted imaging + pattern recognition → Leukodystrophy / metabolic disorder diagnosed. •Acute hypoxic injury in neonates Basic MRI misses subtle findings. With proper DWI/ADC timing and interpretation → Extent and prognosis become clear. •Spine cases Standard screening done → “degenerative changes.” With focused sequences → Cord edema / subtle compression / early myelopathy picked up. •Infections / encephalitis Without proper sequences → non-specific. With DWI, FLAIR, SWI correlation → Pattern points toward specific etiology.

@arvindreddyt @itisprashanth Considering the technicians are operating the machine, I thought its more of a process that needs to be followed to the T. Isn't there a fixed SOP for every type of MRI ordered? Or there are MRIs where the Docs describe the sequence/planes?

If MRI was just about taking images, why do so many patients come with scans — and still no diagnosis? In pediatric neuro alone, I repeatedly see: •Children labeled “normal MRI” → later found to have focal cortical dysplasia •“Delayed myelination” → actually leukodystrophy patterns •Incomplete studies missing DWI/SWI or metabolic clues These are not rare — they are routine. That’s the difference between protocol-based scanning and thinking-based imaging.

@arvindreddyt @itisprashanth I agree with you Arvind reddy💯. The MRI sequences and the expertise behind the reporting is always important. If there aren’t correct sequences and reporting, it’s as good as paying 4000 for a X-ray. This is the reason why patients have dozens of MRI without a diagnosis. (1/2)

@sajanpv @itisprashanth A clinician correlates — but cannot reconstruct missed data. If the scan lacks the right sequences or planes, no amount of clinical skill can extract what was never captured.

@arvindreddyt @itisprashanth So can't the treating doctor interpret the images and clinically correlate? Isn't repetition the mother of all skills? aka Dr. Devi Shetty model?

Acceptance doesn’t equal excellence. Most hospitals accept outside scans for convenience and speed, not as a certification of optimal quality. If something is unclear or clinically mismatched, they still repeat or supplement the study. Acceptance is a workflow decision — not a benchmark of diagnostic depth.

Yes — because you’re not paying just for a scan. You’re paying for: •Proper protocol selection •Real-time decision-making during the scan •Experienced interpretation •Clinical correlation MRI is not a fixed product — it’s a process. That process is what creates the difference.

Getting a DICOM and sending it elsewhere is not the same as doing the study right. MRI quality is determined at the time of acquisition, not after. If the protocol is inadequate, sequences are missed, or planes are suboptimal — no expert radiologist can recreate that information later. A good report starts inside the scanner room, not on a laptop afterwards. Definitely the price is can’t be compared to a diagnostic centre as the cost of setting up is always different from a multispeciality hospital

@arvindreddyt @itisprashanth DICOM (.dcm) format we can get from them and provide it to the most experienced radiologist doctor as well and then provide the good consultancy charges. Even this the difference is not justified

We are breathing poison every single day, and silence from those in power is no longer acceptable. Air quality is not a luxury issue—it is a basic right to life and health. When AQI crosses safe limits, it is governance failure, not just pollution. We demand accountability, transparency, and immediate action—not excuses. Every delayed decision is another day of damage to our lungs, our children, our future. If you cannot ensure clean air, you are failing the very people you were elected to serve.

Highway Harassment: Two Youths Create Chaos Near Shobha Town Park in Bengaluru Some aspects of human behavior, unfortunately, seem difficult to change especially when it comes to a lack of civic sense and respect for others in public spaces. A recent incident on the highway near Shobha Town Park in Attibele highlights this concern. A car bearing registration number KA-03NY-3552 was reportedly seen causing disturbance on the road. According to witnesses, two young men inside the vehicle were behaving irresponsibly, making inappropriate hand gestures, shouting at other motorists, and creating unnecessary chaos. Their actions not only caused discomfort to fellow drivers but also raised serious concerns about harassment and road safety. Such reckless and disrespectful behavior on public roads is unacceptable. Highways are meant to be safe spaces for everyone, and actions like these can easily escalate into dangerous situations, potentially leading to accidents or conflicts. It reflects poorly on the individuals involved and shows a disregard for basic social responsibility and discipline. Authorities should take note of such incidents and ensure strict action is taken against those who misuse public spaces in this manner. At the same time, it is important for individuals especially young people to understand the importance of responsible behavior, mutual respect, and adherence to the law while using public roads. #bangalore #bengaluru #attibele @bngdistpol @blrcitytraffic @BlrCityPolice @alokkumar6994 @DgpKarnataka @KarnatakaCops @Lolita_TNIE @ChristinMP_
