MRI is not a “follow SOP and finish” test.
There are baseline protocols, but real diagnosis depends on what you add beyond that.
From my own clinical practice, this is what actually happens:
•Pediatric epilepsy case
Child comes with “MRI brain normal.”
On repeat with proper epilepsy protocol (thin slices, oblique coronal through hippocampus) →
Focal cortical dysplasia identified.
→ Entire management changed.
•Developmental delay / metabolic suspicion
Routine MRI done with basic sequences → reported as “delayed myelination.”
On targeted imaging + pattern recognition →
Leukodystrophy / metabolic disorder diagnosed.
•Acute hypoxic injury in neonates
Basic MRI misses subtle findings.
With proper DWI/ADC timing and interpretation →
Extent and prognosis become clear.
•Spine cases
Standard screening done → “degenerative changes.”
With focused sequences →
Cord edema / subtle compression / early myelopathy picked up.
•Infections / encephalitis
Without proper sequences → non-specific.
With DWI, FLAIR, SWI correlation →
Pattern points toward specific etiology.

Isn't there a fixed SOP for every type of MRI ordered? Or there are MRIs where the Docs describe the sequence/planes?
If MRI was just about taking images,
why do so many patients come with scans — and still no diagnosis?
In pediatric neuro alone, I repeatedly see:
•Children labeled “normal MRI” → later found to have focal cortical dysplasia
•“Delayed myelination” → actually leukodystrophy patterns
•Incomplete studies missing DWI/SWI or metabolic clues
These are not rare — they are routine.
That’s the difference between protocol-based scanning and thinking-based imaging.

If there aren’t correct sequences and reporting, it’s as good as paying 4000 for a X-ray. This is the reason why patients have dozens of MRI without a diagnosis. (1/2)

Acceptance doesn’t equal excellence.
Most hospitals accept outside scans for convenience and speed,
not as a certification of optimal quality.
If something is unclear or clinically mismatched,
they still repeat or supplement the study.
Acceptance is a workflow decision —
not a benchmark of diagnostic depth.
Yes — because you’re not paying just for a scan.
You’re paying for:
•Proper protocol selection
•Real-time decision-making during the scan
•Experienced interpretation
•Clinical correlation
MRI is not a fixed product — it’s a process.
That process is what creates the difference.
Getting a DICOM and sending it elsewhere is not the same as doing the study right.
MRI quality is determined at the time of acquisition, not after.
If the protocol is inadequate, sequences are missed, or planes are suboptimal —
no expert radiologist can recreate that information later.
A good report starts inside the scanner room, not on a laptop afterwards.
Definitely the price is can’t be compared to a diagnostic centre as the cost of setting up is always different from a multispeciality hospital
@arvindreddyt@itisprashanth DICOM (.dcm) format we can get from them and provide it to the most experienced radiologist doctor as well and then provide the good consultancy charges. Even this the difference is not justified
