on national doctor's day, i was excited to receive my copy of David Lortscher, MD's new book, "Why Doctors Win." the book tells the stories of luminary physicians many of us have studied for years, from Dr. Bud Rose, Founder of UpToDate to Drs. Leonard Schleifer and George Yancopoulos, Founders of Regeneron. what connects these stories is a pattern I think about constantly: medicine is one of very few disciplines where deep fluency in the 1:1 craft, the irreducible act of caring for a single patient, becomes the foundation for building at 1:many scale. the physician who has sat with diagnostic uncertainty, navigated a broken workflow, or watched a patient fall through the cracks carries embodied knowledge, a form of structural advantage no diligence process or market analysis can reproduce. happy doctor's day, to those at the bedside and those building from it!

A few weeks ago the @ycombinator batch begged me to make a CRM crash course for founder-led sales. After multiple drink offers 🍻— I gave in. The session itself went semi-viral inside the batch, and people keep asking for the video. If you’re doing founder-led sales, the CRM might feel like this clunky behemoth system—it doesn’t have to. JAMA Psychiatry, 2025: Machine learning is not (yet) ready to predict the onset of schizophrenia or bipolar disorder. Few interesting points: ✅ (1) Schizophrenia prediction outperformed bipolar prediction. Why? Schizophrenia onset is often much more uniform, while bipolar onset is more heterogeneous, making ML's job that much harder. ✅ (2) Only routine EMR data was used; no new data was collected. Interestingly, only the free-text had predictive value. Including the structured data did not make any meaningful improvement. ✅ (3) Positive predictive value (PPV) was too low, across the board. Even though the model showed good discriminatory power for identifying potential cases of schizophrenia (AUROC = 0.80), it wasn’t very good at being right when it made a positive prediction (PPV only 10.8%). This highlights a common challenge in low-prevalence conditions—high false positives despite good overall discrimination. Much more wood to chop here! ## 📚 TL;DR: Earlier diagnosis is critical. But ML is not ready for this yet. We need more data, better models and more computation horsepower. And with the rapid pace of improvements on all of these fronts, I have no doubt we'll see this solved, soon! ##
