heya @bcherny since around the time auto-dream and Auto Mode launched, which I both use, Plan Mode often just automatically executes after proposing a plan, without waiting for my approval. Similarly, often when I'm asked questions, it skips ahead and goes "blank answer provided". This is in the CLI, with --dangerously-skip-permissions
