Lesson 1. Resuscitation is the easy part of caring for critically ill patients. Resuscitation or ‘rescue’ is one of the most exciting parts of the job, however, is only a fraction of what’s needed to care for a critically ill patient. Without being competent at resuscitation, the other aspects of patient care suffer, but hyper fixating on resuscitation can be at the detriment of patients. After some time (typically hours), care needs to shift from resuscitation into stabilization. The things that may have benefited patients early on (e.g. fluids) may harm the patient later on. Importantly, the intensity of resuscitation often corresponds to the intensity of deresuscitation required.

13/ Learning points: -POCUS ftw! - Go to DS defib early in refractory shockable rhythm. By the second unsuccessful shock start thinking about. More emerging data on its efficacy -Consider stellate ganglion block beyond that. Get good at guiding your needle to do this quickly.

1/ Wild cardiac arrest with #POCUS playing a major role An elderly male was sent from the nursing home in cardiac arrest. Per the prehospital report that we got, the patient was in vfib and refractory to 5 defib attempts. The patient got amio as well as several doses of epi.

A family member may be at the end of life & the consulting nephrologist told family members that correcting hypernatremia is not necessary for comfort 🤯 It’s one of the most uncomfortable issues that a patient can face & should be corrected, esp while already in the hospital! 🤦🏻‍♀️
