HALLUX VALGUS WITH LESSER TOE VALGUS AND METATARSUS ADDUCTUS IN 52F PROCEDURE: 1st metatarsocuneiform arthrodesis with fusion of bases of metatarsal 1 and 2, medial eminence resection, distal soft tissue release, and medial capsular imbrication (Lapidus); Akin osteotomy, Proximal Phalanx Varus Producing Osteotomies for toes 2, 3 and 4, with no internal fixation, just post-op toe taping. @orthotraumamd @orthobullets If there is any residual motion either rotational or translational after fusion of the MTC. Will stabilize intercuneiform and NC instability which I have a difficult time sussing out. I think it also helps to further close down the IMA and helps to get a spot weld at 1/2

@orthobullets Similar case I did with met adductus and dislocated 2nd MTP. Was able to push the 2nd and 3rd over by stabilizing the medial column.

@DrPayItBack Happiness in documentation is understanding it principally has two purposes, billing and medicolegal. After you realize that you stop correcting dictation errors and care about prose and write a billable note. Surgeons figured this out long ago

I’m really interested to know how many people outside of billing and coding actually read my notes, even though I already know the answer.

@txsportsdoc How on earth anyone can go into these specialties when the advanced care providers in dermatology and surgical specialties are making more than you is beyond me. Stark laws have been used as a cudgel to break the backs of primary care physicians.

@bemooremd You can’t incur $200-$400k in student debt as US MS then come out and make $195k as an FP(avg in TX). Must make $120k/yr in Dallas now to qualify to buy a house. The math just doesn’t work anymore. I value our prof and just wished some org was working to ⬆️ docs pay.

@txsportsdoc Totally agree. Medicine has been hollowed out and the trend is downward. Probably not going to be addressed without some kind of nationwide strike.

@bemooremd Dangerous precedent IMO. No one wants to go into certain specialities because pays so poorly(manufactured by INS co),shortage created therefore H1B visas to non citizens to fill spots,further driving down what that specialty paid. Much of the unfilles/shortages solved if ⬆️ pay
