The most common and well-known type of corruption. There's even a university course on it at Harvard.

You said the answer. Urgent care. My insurance had very specific cost for non emergency care. My copay at the Emergency room was $200. If I was admitted, it was all covered. If I went to Urgent care or my GP office (which is hard to get an appointment at), it was $50. Emergency rooms are notorious for high cost because they, by law, must take anyone who walks in the door whether insured or not and have to eat the cost of the uninsured.
However, she is 100% correct in that Insurance companies have quietly become monopolies that now own Pharmacies, Doctors practices, Hospitals and Urgent cares. They own every facet of your health care so there are no checks and balances. They also own most politicians through political donations and board appointments after they "serve the public" (I use that term sarcastically) The so called ACA (Obamacare) was an open door to this boondoggle.
