so you’re telling me 5 min before trump announced a ceasefire, someone in his inner circle loaded up on a perfectly-timed trade (long stocks, short oil), walked away with billions, and then we the plebs are told $8 gas is just “the price of freedom” so just get less starbucks ???
unusual_whales@unusual_whales

BREAKING: Just five minutes before Trump's announcement to halt the attacks on Iran, massive trades reportedly hit the market. In one move, $1.5 billion in S&P 500 (ES) futures was bought while $192 million in oil (CL) futures was sold. These orders were 4–6x larger than anything else at the time. The trader seemingly made huge gains. Unusual.

@northwoods1980 Initial non-op management is appropriate. Consider surgery for recurrent instability, inability to return to sports, contact athletes, and throwers.

Management quest for a 13-yo with first-time traumatic anterior shoulder dislocation and Perthes lesion? I assume this will heal, scar down just fine?

@northwoods1980 I don’t have any issue with not resurfacing the patella. That’s a game time decision based on the appearance of the patella. I do have reservations about press fitting a total knee in a 92 year old, but would defer to the surgeon based on bone quality. It’s not contraindicated.

Any thoughts, comments, opinions regarding a non-cemented arthroplasty and un surfaced patella in a 92-year-old with osteoporosis?

He’s on his way to the hospital for an angiogram to confirm no vascular injury and likely compartment monitoring, as vascular injury and compartment syndrome are the two most immediate threats to limb viability.

Just from the way he planted and the appearance of his leg after the play, #Tyreek looks like he most likely dislocated his knee and it was immediately reduced on the field. These are devastating potentially limb-threatening injuries, true orthopedic emergencies.

@jointdocShields I use it for my totals and have had no issues. My first several throws suprapatellar are interrupted #1 vicryl pop-offs, then #1 stratafix for the remainder of the arthrotomy.

Anyone else use Stratafix suture for closure (ethicon)? And have you recently noticed a higher rate of suture breakage when closing?

@northwoods1980 I prefer BT in almost all populations. I think doing less work in the joint is probably best. Also, they seem to have less pain and get motion back much faster.

I appreciate this work. Helps inform, tailor the way I read shoulder MRI exams. Also in older populations. The tide, trend has really changed over the past several years.

@RepGregMurphy @GmOrr9000 Refund the federal government? You realize that the federal government doesn’t pay medical school tuition for 99.99% of the physicians in this country, right? Only those enlisted.

@GmOrr9000 It should be mandated that unless disabled, each medical school graduate should have to practice medicine FULL TIME for 15 years (minimum) or have to refund the Federal Government for the cost of tuition.

#1 reason for MD shortage is Med schools admitting students who aren’t going to practice. Only 60% of med students today plan on practicing clinical medicine. Med Schools need to focus on admitting students who want take care of patients, not just get their MD.

It’s always so surreal seeing these monsters offshore, so close and so devastating. I’m wishing all of my friends and colleagues in Northern Florida and the panhandle a safe ride. Evacuate if possible, but if not, please stay safe.

#orthotwitter - How do you decide how much call a person in your department takes? Age? Years of service? How do you decide when a person can leave the call pool? Age? Years of service? Something else for both?
