@bvbal79 Could be brain dose of 5 to 10 cGy in certain settings. And over 30 fractions that adds up and COULD plausibly have an effect. I stress could. These are CT scan (or maybe more) similar doses, and CT scanning has been associated with Alzheimer improvement pmc.ncbi.nlm.nih.gov/articles/PMC48…

Conventional 1.8-2.0 Gy per day is reasonable for those who only know how to design an APPA or 4-field box, or don’t know how to properly aim a linac at a target. @aleberlin2 @alison_tree @nickva1

Why isn't organ preservation more of a thing for esophagus cancers (other than upper SCC)? In CROSS, pCR was 49% for SCC, 23% adeno - and that was with 41.4 Gy. With higher dose of RT, addition of IO, and longer interval to assessment, we could probably improve upon complete response rates - close to what we can get in rectal cancer. At what pCR rate should we be entertaining idea of organ preservation in GI tract cancers?

@NiuSanford Ironically though, I looked at over 10k deliveries with ICDs and 2 devices needed to be re-programmed. Both were in prostate cancers so not bad to consider effects far from field.

I get the study is comparing IMRT vs. 3D radiotherapy for prostate cancer, but perhaps should at least mention baseline rate of new solid malignancies at 10 years in men >65 years without any RT to put into context. My guess is it’s also around 7%. jamanetwork.com/journals/jamao…
