Minnesota’s nonprofit hospitals posted $2.4b in net income, which saved them $504m in federal taxes. But they devoted just $163 million to charity care. This means that for every $1 taxpayers subsidized these hospitals, they got .32cents in charity care. That’s a bad deal. The math gets worse if you add the state’s 9.8% corporate tax rate. We absolutely need to worry about the future of specialty care. If I do a craniotomy for a TBI at 2am, the payment system now financially rewards me more for billing the ICU care afterward than for performing the actual surgery itself. We can increase primary care reimbursement without creating these kinds of central planning distortions.

Top-tier #MedTwitter click bait in @Health_Affairs. Here's the thing: 1) We don't need to worry about "sacrificing specialty care." The average specialist makes >$400K. 2) It is entirely reasonable to evaluate the impact of recent payment increases before doing more.

Half patients required parenteral nutrition, their patients didn't get to 50% of their needed calories until day 5-6... our patients are at home by then.
