A patient has a cardiac arrest with chest compressions ongoing. A resusTEE is performed. Despite the CPR being done hard, fast, and with perfect recoil, why will this patient die despite everything being done?👇 (that is, unless you fix)

Can anyone explain the need for an extra "break the glass" in a patient chart in the electronic health record? It's already illegal to enter a patient chart if you're not caring for the patient, so why do some patients deserve "extra" security?

@simontutt88 @jbrannan23 I am open to be corrected here. IGel is the first line option for advanced airway, but if that fails and basic airway is challenging too, what’s the harm of having a not-so-competent intubator try? The patient is hypoxic in either scenario.

@conpapak @jbrannan23 I suppose the issue is gaining and maintaining competence to perform. I was lucky and did 58 tubes in undergrad back in those times and since than have done anaesthetics + list bimonthly and sims alternate months using SPA, could we extend that to all paras. Likely not possible

Today SAS removed endotracheal intubation as a core skill for paramedics. APCC and aircrew paramedics will maintain the skill. I know this is an evidence based act. But, for my own curiosity, I wonder if the evidence takes into consideration rural and semi-rural communities

Interested to hear from students. On paramedic courses how much training are you getting on breaking bad news. Mine had 2 slides on bad news, and 2 days on maternal emergencies. 46 times I’ve had to tell family their loved ones have died in 12 months, and I’ve been to 3 births

@TheLeeMcLaren A classroom gives you a safe space to focus and build your knowledge. Placement then compliments that with the hands on approach. Paramedicine is taught in the classroom and practiced on the streets. You need to know what to do before you go out and do it.

@moribunddr Wouldn’t call it micromanagement. It’s the pressure filtering down unfortunately. I can imagine that in those 15 mins, they’ve had ten bleeps themselves from discharge coordinators, bed managers, matrons, PTS booking managers etc. Don’t blame the nurse, blame the system.

@Ryanharris2021 I did a PGDip in clinical education as NQP and a couple years later started a critical care MSc. On both courses, I had colleagues who struggled to adapt to academia after gaps with their studies.

@Aidan_Baron @EdinburghUni has been offering HCP-Med for a couple years now. It’s former of 3 year part time (while still working as a clinician), followed by 2 years of full time studies. Brilliant idea and would love to see than expanded across the whole of the U.K.

In Europe medical degrees take 6 years. In the US medicine is a postgraduate 4 year degree. How can the UK do a 4 year undergraduate degree? British exceptionalism?🤷‍♂️

@rosiedoddjones Most arguments have already been made, but I’d also highlight the analgesic properties of Aspirin. Used a lot more as an analgesic in other countries and is part of the care bundle for ACS.

Calling #MedTwitter! Paracetamol in cardiac chest pain in the normotensive patient. Yay or nay? I usually give Morphine in isolation if pts respond well and seem to remember that paracetamol is not recommended in cardiac chest pain. Can’t find any literature though?

@FFAFrances @DaveSwart7 @ParamedicsUK Indeed, a prefilled syringe (posiflush) is actually a medical device thus anyone can use it. If you draw up sodium chloride for the purposes of flushing a line or otherwise, you are using the S17 exemption which is only valid for registered paramedics.

@DaveSwart7 @ParamedicsUK Students can give certain drugs. I was able to give some oral. Sublingual, inhaled and IM injections. What confused me as a student was that I could cannulate but wasn't allowed to flush it through to make sure it was patent as that was considered drug administration through IV.

I was told last week that UK paramedic students are not permitted to give ANY drugs whatsoever until they are qualified. Is this true and if so, why? Medicines governance? I have opinions but just trying to understand it first. Apparently this is being supported by @ParamedicsUK?
