PCOS/PCOD is not just a “hormonal disorder.” It is a metabolic-endocrine condition driven largely by insulin resistance. Simple tips to improve PCOS (evidence-backed)👇 1) Improving insulin sensitivity is the primary target Lifestyle interventions consistently show: ✔ Reduction in fasting insulin ✔ Reduction in serum testosterone ✔ Improvement in ovulation rates ✔ Better menstrual regularity 2) Nutrition: focus on glycemic load, not just calories High glycemic foods → rapid glucose spikes → exaggerated insulin response ✔ Low glycemic / high fiber diets shown to: • Reduce insulin levels • Improve androgen profile • Improve cycle regularity ✔ Protein-rich meals: • Reduce postprandial glucose excursions • Improve satiety → indirect weight control 👉 Mechanism: flatter glucose curve → lower insulin demand 3) Weight loss (when indicated) Even 5–10% reduction in body weight leads to: ✔ Improved insulin sensitivity ✔ Decreased ovarian androgen production ✔ Restoration of ovulatory cycles 👉 Small fat loss → disproportionately large hormonal benefit 4) Exercise: independent metabolic effect Exercise increases GLUT-4 mediated glucose uptake (insulin-independent pathway) ✔ Results: • Reduced insulin resistance • Lower circulating insulin • Reduction in visceral adiposity Best outcomes seen with: → Combination of resistance + aerobic training 5) Adipose tissue is hormonally active Excess adiposity (especially visceral fat): → Produces inflammatory cytokines → Worsens insulin resistance → Contributes to androgen excess 👉 PCOS is also a chronic low-grade inflammatory state 6) Sleep & stress: underestimated drivers Chronic stress → ↑ cortisol → Worsens insulin resistance → Disrupts hypothalamic-pituitary-ovarian (HPO) axis Sleep deprivation: → Impairs glucose metabolism → Increases hunger hormones (ghrelin) 👉 These directly worsen PCOS phenotype 7) Why “quick fixes” fail OCPs may regulate bleeding But do not address: ❌ Insulin resistance ❌ Underlying metabolic dysfunction 👉 Symptom control ≠ disease reversal Clinical Strategy: Reduce refined carbohydrates & added sugars Prioritise protein + fiber in every meal Structured exercise (≥150 min/week) Target visceral fat reduction Optimise sleep (7–8 hrs) Manage stress physiology Intermittent fasting (to reduce insulin exposure window) PCOS is not “cured permanently” But it can be driven into remission with sustained metabolic correction PCOS is not primarily an ovarian problem. The ovary is responding to a metabolic signal. Fix the signal → the system corrects itself.
