I'll add that the region of North America (excluding Mexico and Central America) that has/had the highest amount of incest isn't West Virginia or the Appalachians. It's Quebec. The initial European colonists were primarily men and they solved the woman shortage in different ways. English colonists either brought their wives with them (as in the case of the Puritans) or sent for women back home (more common in the South and Mid-Atlantic states). In the latter case, this was done through indentured servitude, where the women would marry other colonists. Occasionally, some English would marry Indian women, which is why a small number of white Americans have American Indian ancestry (and why non-trivial numbers of Indians have white ancestry). The Spanish and Portuguese solved their woman shortage by marrying indigenous women. The French solved it through prostitution (how stereotypically French) and cousin marriage. Incest among the Quebecois worsened after the Seven Years' War. When France ceded New France to Britain, most of the colony's upper class returned to France, leaving the working and middle classes behind. This has left a legacy of inbreeding-related disorders in some parts of Quebec.
