@JerroldLevy I think TEG is more sensitive than traditional coagulation tests and D-dimer. Anecdotally, I have observed surgical patients with GNR septic shock have prolonged R-times with no other TEG abnormalities and normal PTT and INR. >50% mortality so very acutely ill subset of SIC pts

@deRouletMD Interesting question- plasma has many potential uses- likely also dependent if DIC. Viscoelastic testing in my view for DIC/SIC evaluation is too insensitive. There are several scoring systems for SIC/DIC but none use VET including TEG.

@JerroldLevy Thank you for MGH lecture today. I thoroughly enjoyed! Question: is there a role for [solvent detergent] plasma resuscitation in early course of GNR sepsis induced coagulopathy based on TEG (high r-time). Is high volume crystalloid resuscitation worsening SIC?

@Kniery_Bird @jmills1955 @DanHan @VascularMD We were doing that last week but found groin lines were interfering with proning. Now it’s standardized (we have 70 intubated patients so far and 500 in house with Covid): Left IJ Triple lumen Right IJ saved for shiley (about one third need) Rt radial a line > rt fem

Thoughts on femoral central venous access and arterial line access as first line access in high volume COVID hospitals? Less draping, faster, easier access, lower complications, maybe equivalent risk of blood stream infection? Higher DVTs? @jmills1955 @DanHan @VascularMD

@NYGovCuomo What are we doing for compensation for the frontline teams? As a resident physician, I am asked to provide frontline care but am making ~$10/hr. Is any part of the stimulus directed towards compensation for frontline providers and resident physicians? We deserve more.

@realDonaldTrump @AmCollSurgeons If residents and fellows are being asked to staff Covid ICUs it's ridiculous that we are getting paid ~10/hr. We are putting our lives on the line. This is not education, this is service. Those of us on the frontline deserve better compensation.

Very interesting data, highlighting that with more testing across the US over the coming weeks and good public health preventive practices we too can bring our mortality rate in the US of #COVID19 down and close to that of S Korea.

Study of 34K pts: Surgical jackets & bouffants are neither beneficial nor cost-effective in preventing SSIs. Institutions should evaluate their data to determine whether recommendations by outside governing organizations are beneficial and cost-effective. ja.ma/37kS4Qz
