@helenvecht @catherineroyuk Assessing for anal sphincter injury should involve a PV and PR examination. When there is profuse bleeding and the vulva/vaginal tissues become quite oedematous in labour, it can be difficult to diagnose and restore full function.

Is this failure to LOOK at the perineum?
I accept there’s much blood & gore just after delivery but it seems nobody looked...

An anecdote of an interaction with a patient that has stayed with me for 15 years.
When I was a fresh-faced 2nd year doctor in a GP practice, a young lady came for an appointment.
She couldn’t have been more than 5 years older than me.
The reason was ‘work stress’

Nurses are super protective of their registration & much less likely to put it at risk by doing anything outside their scope!
