@Drfattyliver @IanARowe Age is a strong predictor of waiting list mortality so to maximise population survival older patients with same disease severity are prioritised. This was an anticipated outcome of the scheme. Post LT survival whether to 5 or 10 years doesn’t compensate for that. We’ve tried.

I look forward to see data on NLOS 2.0 @IanARowe @dougton. Age continues to be a huge factor in the offering system. Our are younger high ukelds live in purgatory with bili >200-300 with very few offers. No system is perfect, would love to see how offering changes w 10yr outcomes

@aelsharkawy75 @IanARowe @Drfattyliver @EASLedu Scheme introduced in 2018 and several years of data on impact (covid disrupted onlinelibrary.wiley.com/doi/full/10.11…) produced for independent national liver offering scheme (NLOS) monitoring committee chaired by @IanARowe reviewing data 3 monthly now 6 monthly. NLOS 2.0 on way.

@AutoImmuneLiver @Drfattyliver @IanARowe Yes easy to blame system when fundamental issue is discrepancy between supply and demand. Solution is increased utilisation marginal dbds and dcds supported by nrp and perfusion technology hand in hand with LDLT.

@aelsharkawy75 @IanARowe @Drfattyliver @EASLedu In UK we uniquely offer dbds nationally on transplant benefit score (TBS); maximising the difference in survival at 5 years between anticipated survival with LT with that organ v. survival without LT so that utility is considered in addition to need. 21 recipient 7 donor factors
