Important new @AnnalsofSurgery study from Japan focused on the impact of rectal cancer surgery on male sexual function. Important points: -Excellent response rate (95+%) with inclusion of preop as well as longitudinal postop data. -Only 6-7% had preop radiation, and BMI was much lower than what's encountered in the US. -Across all approaches (lap, robot, TaTME), preop rates of erectile dysfunction were pretty high, got much worse after surgery, and had moderate improvement at 12 months postop. -Benefits of robotics may have been blunted by surgeons being early in the robotic learning curve (27% had <20 cases, and 60% had <60 cases) journals.lww.com/annalsofsurger…

When closing the abdominal fascia, the most common tool used to protect the underlying bowel from getting poked with the needle is probably the malleable retractor ('ribbon', if you prefer). Normally the wide one on the L is used, and you switch to the narrow one near the end.

11 small things that I do to bring a smile :- —————————————————- 1. Whenever I buy something from a shopkeeper ( especially the small ones ). Pay them off to the higher rounded zero. Example :- If the bill is 14 Rs. I pay 20 Rupees. 2. To every single person who comes to my home to deliver anything I offer them a small water bottle. 3. Every Diwali 🪔 I buy sweets for all the security Guards & labourers in my apartment. 4. I give minimum 100 Rs. tips to parking guys at restaurant and many other places. 5. Whenever I buy some sweets or snacks I buy one set for my driver too. 6. I always ensure that my cook flies to his hometown once a year at my cost 7. I use a porter in Railway Station even if I’ve only one bag. 8. If i travel in Ola / Uber cab service I always ask them to start a SIP with the “first month paid by me offer”. 9. G-pay min 501 Rupees to any temple Pujari. 10. Drop my window down and advise anybody who has a child on a two wheeler without a helmet. Have got abused and appreciated by multiple folks. 11. Suddenly call my office boy / security guard / any labour staff who have been honest and sincere and ask them a surprise question ? What would they like to take home today ? Or how would they like to spend this Sunday ? Many of them have said they would want to watch a movie and have lunch in a hotel of their choice with their family…. I make that happen. ……. The reason for me to share this is only one :- If any of you think, one or many of them are worth emulating. Pls do. It brings a smile to their face beyond your imagination. The joy it gives them is unbelievable and it is truly a beautiful feeling to witness that. #SpreadKindness #SpreadLove #SpreadMagicInEveryStep #letuashare #joy #wecare

Long List of things gone from our life ———————————————— 1. We don’t visit others' houses JLT 2. Have forgotten going to relatives' homes for good. 3. Friends don’t meet until it is planned well in advance with a strict log-out time. 4. Very few folks to be at your side even during a crisis as grave as death of a family member. 5. The habit of Strolling across the road for a cup of tea is gone. 6. Nobody is surprising anybody by a family visit. 7. Cousins are never staying over at each other's place. 8. Elders used to play underarm cricket/ carrom/chess/ludo with us - that’s gone … elders rarely are to be seen. 9. During marriages people used to spend hours chatting and joking and having fun…. Today it is visit and scoot 10. Festivals were together and big …. today it is pic sharing of pixel size family 11. Sending food and other stuff to neighbors used to be regular - now it’s non-existent 12. We have adopted the method of calling and fixing appointments to meet friends 13. Tc / RIP / Tq / Cya / HB / JLT …… everything has shortened. Not just English but our time, availability, and just pure care. I've decided to do something about it. I've decided to: 1. Bump into people’s homes without notice 2. Go to relatives' homes without calling 3. Meet a friend JLT 4. Walk into a neighbor's home and sit down for a few hours 5. Take my children to cousins' homes 6. Meet elders and call them regularly over the phone 7. Make family WhatsApp groups active and lively 8. Make school friends' groups more vibrant and engaging and honest 9. Make life more natural and less appointment-based 10. Call clients home as friends and not as business mates 11. Ask for the numbers of some Twitter friends and call and just speak to them…… no agenda … just plain friendship and chatting …. Let us rise up and reclaim our lost connections! Let us revive the art of human touch, of spontaneity, and of love. Let us make a conscious effort to create moments that truly matter. @DCRjournal @CelestinoGutirr For classic splenic flexure tumours, a classic radical left hemicolectomy, taking the left colic, left br MCA and the IMV from its root. Transverse to proximal sigmoid colon anastomosis. If tumour extends either more proximal or more distal, then a subtotal Colectomy.
