Ended the week with a great PEN FLASH 3.0 case. Up and over and able to treat the infrapopliteal confluence and the saphenous collaterals. Went PTV and flossed from above (felt like an arterial case!) Treating the inflow may shift the paradigm! venous disease experts need to be well versed in multiple techniques/devices. PeVD - when reflux is this florid, I’d be hard pressed to argue closure isn’t indicated - unless it represents the only renal vein outflow (even in this case I’m told of how VS ligate renal vein during open aortic repairs at times without ill effect) Many women live with symptoms of this disease and go undiagnosed, we need better standardization of diagnostic and treatment algorithms for this very real and QOL affecting disease.

What's going on here? Why did we place an IVC filter for a patient with Acute Limb Ischemia? Every vascular interventionalist will eventually encounter this condition: *Paradoxical embolism* A clot from the venous system ends up in the arterial system through a right-to-left shunt 🫀 After IVC filter placement, arterial lytic + aspiration was performed, with a good outcome. Keep this condition in the back of your mind when you consider acute limb ischemia causes! 🧠
