@drsimonfeng⁩ now I’m sure you’re going say well that was 2008 and that was a different country, but does it actually matter? She’s saying many prefer Subutex to heroin. it’s incredibly myopic to just conclude it’s non-addictive and irrelevant, even if people are addicted

@Wanita1 @JGillespie67 @ibdgirl76 @CMerandi It appears bup could be addictive in some environments, esp where no other opioids available. Fentanyl users can’t use bup — puts them into precipitated withdrawal. IMO most street bup used to self-medicate withdrawal. I’ve never seen a “bup addict” who wasn’t already addicted

I would like to ask that you post evidence that Buprenorphine isn’t addictive but it’s much more difficult to prove a negative, is it not? Would it be true to say that Buprenorphine, as a partial opioid agonist (not sure if I said that right), might carry a risk of addiction tho there is no definitive proof to support the statement? Bc it binds to opioid receptors, should we not hypothesize that Buprenorphine carries a small risk of addiction? At least until it’s proven otherwise? Just to be on the safe side? Pts can be properly informed regarding no evidence about whether or not it is addictive.

@JGillespie67 I’ve Rx opioids for CPP for 20+ yrs. I teach residents not to be afraid of Opioids. I give lectures on the safety of opioids. I never force bup on CPP. Look at my old posts to see my posts on CPP. But you’ve made up your mind so I won’t try to convince you.

It sounds to me like the buccal form is responsible for ppl losing their teeth. I’ll need to look it up. I have seen on Twitter, ppl who refuse to take this medication for pain bc of misinformation. My brother suffers from Arachnoiditis in his neck which causes nerve pain down his arm & Butran’s patch worked to treat his pain. The problem was tolerance.

@james_stampfer @ibdgirl76 Disease & choice are not mutually exclusive. Type 2 diabetes related to lifestyle choices; COPD from choosing to smoke. We choose behaviors not consequences. Addiction is like COPD, the consequences of unwise choices, not the choice itself.

@drsimonfeng @ibdgirl76 I’ve just briefly checked out some of your work and although I agree with some of your sentiments regarding abstinence based approaches, I find the disease model falls short of reality. It’s a learned behaviour with functional neural adaptation. A disease does not provide choice.

I never one time claimed that since there is a small risk of addiction, it shouldn't be prescribed. That was all you needing to promote a narrative. Patients deserve truth. It’s interesting how you say not enough is known about the addictive potential of Kratom but you don’t say that about Bupe. You simply say there’s no evidence of it. This is the exact hypocrisy that I struggle with when it comes to addiction medicine. The drug exceptionalism is wrong.

@ibdgirl76 @Whritenour1 @JGillespie67 @CMerandi Kratom is poorly studied. Bup has been studied intensively for decades. Bup abuse is common & often confused for bup addiction, just like opioid dependency is mistaken as addiction. You’ve convinced me bup may be addictive in certain environments.

Bupe addiction is a particularly Finnish problem as more than 80% of high risk opioid users have used bupe as their main drug. "

Did I say it was due to bupe addiction? No, again this is you spinning a narrative. You claimed there have been zero fatal overdoses, and I showed you studies proving otherwise. Many who did from illicit fentanyl aren't opioid addicted either. in fact, those who aren't have higher risk of OD.
