Transscleral IOL Fixation: When the Capsule Is Gone but Vision Is Still Salvageable Loss of capsular support remains one of the most challenging scenarios in anterior and vitreoretinal surgery. In these cases, transscleral intraocular lens (IOL) fixation becomes a powerful option to restore stable visual rehabilitation. Let’s review what matters clinically. When Do We Consider Transscleral Fixation? This technique is typically indicated when conventional capsular support is absent or insufficient. Common situations include: •Aphakia after complicated cataract surgery •Late IOL dislocation •Trauma with zonular dialysis •Lens subluxation in conditions such as pseudoexfoliation or connective tissue disorders •IOL exchange when capsular bag support is lost The main goal is stable IOL positioning while minimizing long-term complications.

Finally published: the COBRRA trial, the first randomized head-to-head comparison of major DOACs — something the companies would never have done themselves, as a direct confrontation goes beyond their commercial interests. This was driven by independent investigators. The results somewhat support the idea of apixaban being the “safer” DOAC. That said, the list of study limitations is long, and for me, labeling something as “safe” is not enough — after all, even placebo is “safe” when it comes to bleeding. nejm.org/doi/full/10.10…

Streeting refuses to apologise, talks down to doctors, then implies he’ll tone it down if they stop striking and accept a weak deal. That isn’t constructive dialogue or good faith negotiation. It’s disrespectful pressure and it explains why trust in this process is so low.

The fact that the Doctors Vote 🦀 people keep ignoring emergency legislation for prioritisation of UK-trained medics for specialty training places shows this is wilful misrepresentation of the offer and its impact - reducing competition for places from 4:1 to *less than* 2:1.
