@ASHP_EMPharm We just added back to formulary earlier this year and restrict to the ED. Depending on indication/dose we require an EKG.

@LRayRx @ASHP_EMPharm A long time ago 1000mg IM was given before I arrived in the ED. Patient was out for 8+ hours and somehow we did not need intubate

@ASHP_EMPharm A4: I think 3-5mg/kg is appropriate and very effective in many ED scenarios. Familiarity and monitoring is essential. I witnessed a fatal case brought in CPR in progress; 500mg given to a 60kg person in the field. Pt was under-monitored in field and side-effects realized too late

@ASHP_EMPharm Yes we use 4 mg/kg for violent patients. Works well but we do see a lot of desaturation due to hypersalivation/laryngospasm so monitoring is important.

@EMRxJaxson Started ECMO in a trauma bay yesterday... stayed in ED for 5 hours. 110 patients in a 57 bed ED.... I don’t even know. The boarding problem is so real.

Never thought I’d start an inhaled epo and paralytic drip in the ED, but here we are with the hospital at capacity, 75% of the ER patients are boarding, and you have a severe ARDS patient come in #EmergencyMedicine
