Lumbar spinal stenosis is primarily a clinical diagnosis rather than a radiographic one. Even in the presence of radiographic changes, considerable uncertainty remains regarding whether laminectomy +/- fusion provides benefit for people with neurogenic claudication, and if so for whom, and when. In this paper we expose the paradoxes that emerged with the revision of the IASP definition of pain - mainly the addition of the ‘resembling’ criteria, and we offer ways to resolve that.
