As a young researcher committed to the fight against antimicrobial resistance (AMR), I have come to realize a simple truth: the most powerful strategy against resistance is preventing infections before they occur. In clinical practice, we often rely on broad-spectrum or empirical antimicrobial prescriptions, not out of negligence, but out of necessity. Limited access to culture and susceptibility testing especially in low-resource settings forces clinicians to treat based on probability rather than microbiological evidence. Without timely laboratory data, we remain unaware of the local resistance patterns, circulating pathogens, or the susceptibility profiles of bacterial isolates we are trying to treat. This reality perpetuates misuse and unnecessary exposure to antimicrobials, accelerating the evolution of resistance. However, AMR does not begin in the laboratory. It begins with every avoidable infection. When healthcare-associated infections (HAIs) occur, they not only harm patients but also increase antimicrobial consumption and fuel resistant organisms in hospital environments. A simple lapse such as poor hand hygiene, inadequate aseptic technique, or environmental contamination can translate into prolonged hospitalization, medical complications, and the spread of multidrug-resistant organisms (MDROs). This Antimicrobial Resistance Week serves as a reminder: •Policy makers must invest in microbiology infrastructure and surveillance systems. •Researchers must continue developing evidence to guide stewardship and infection prevention programs. •Clinicians must uphold the principles of infection prevention hand hygiene, aseptic technique, environmental disinfection, and responsible antimicrobial prescribing. For me, the frontline of AMR is not only the pharmacy shelf or the microbiology bench. It is clean hands, sterile procedures, and a health system that prioritizes prevention. Because when infections are prevented, antibiotics are preserved.
